IMPORTANT INSTRUCTIONS
1) The application forms should be filled up in Block Letters
2) The application form should accompanied by 2 set Xerox copies of following certificates
A) SSLC Marks card
B) PUC/10+2 Marks Card
C) Conduct certificate
D) Transfer certificate
E) Physical fitness certificate
F) Eligibility certificate from Rajiv Gandhi University of Health Sciences Bengaluru (only Students from states other than Karnataka)
G) Migration certificate (Only Students from other than Karnataka)
H) 5 Passport size Photos
3) Submit all original certificates at the time of admission which will be returned after completion of the course.
4) Fees once paid will not be refunded and any student discontinuing/leaving before completion of course should pay the fee for entire course.
5) Attendance should be maintained, and it should be 80%. As per the RGUHS Norms.
6) All the students maintain discipline both in college and city premises. If he/she fails, they will be punished.
7) Participation in cultural & sports activities is compulsory for all.
8) Use of mobile in college premises is restricted.
9) Disturbing his/her study period, if found rag, such a student will be discharged from college.
10) Payment made outside or inside without Receiving the receipt will not be considered as a payment towards the college
11) Payment should be made through cheque/DD in the name of Tapovana Ayurvedic Medical College & Hospital
